Here at the Gold Medal Swim School, your swimmer is our priority. If you’re taking a break, moving, completing our program, or stopping for health reasons, please complete this withdrawal form.

Form Purpose:
For withdrawing from ongoing lessons (Baby Swim, Learn to Swim, Adult Lessons, Swim Teams, Olympic Development, Semi/Private lessons, or Gold Medal Swim Club). Not for short-term programs such as Boot Camp or Summer Swim Team—please email us for those.
Absences vs. Cancellation:
For short breaks or vacations, log absences in your UDIO Customer Account. You’ll still pay tuition, but your spot and makeups will be held. Canceling releases your spot.
Notice Required:
• Swim School: 2 weeks (2 class periods)
• Swim Club: 30 days
You may choose any future cancellation date.
Makeups:
Book makeups in UDIO before your last day. Credits can be shared within the family or used upon re-enrollment if still valid. Makeups cannot extend enrollment or be redeemed for cash.
Processing:
Cancellations are accepted only through this form. You will receive an email confirmation, then a notice with your final class date. Your spot will not be held—re-enrollment depends on availability, and many classes have waitlists.
Tuition & Fees:
• After the notice period, tuition stops.
• If your last class is mid-month, tuition is prorated.
• Requests before the monthly debit run are charged only for scheduled lessons; after, any balance becomes Family Credit (except for Club swimmers).
• No refunds for past lessons.
• Annual registration fees apply per calendar year; re-enroll within the same year to avoid re-assessment.
