Gold Medal Swim School is built on a unique foundation of Core Values: Safety, Air Regulation, Relaxation, Body Position, and Propulsion. This foundation is introduced in Baby Swim and extends through Learn to Swim, Swim Team, and Adult Lessons. Our curriculum, class structure, and Progress Evaluations reflect these Core Values.

When a student is ready to graduate from a level, we are so always excited to inform you of the news! If you would like a detailed report of your student's progress throughout the year, you may submit a Progress Evaluation request.

You may request a Progress Evaluation for your student once per quarter (once every three months). If you have already entered a request for this student within the last three months, we are not able to fulfill your new request. Progress Evaluation requests are ONLY accepted online; we do not accept phone or in-person requests. Upon submission of this request form, you will receive an Automatic Reply email confirmation that your Progress Evaluation request has been received.

Due to limited time during each lesson, we are only able to accommodate a certain number of Progress Evaluations requests during each class period. We cannot accommodate same-day Progress Evaluation requests. If your request is submitted late at night prior to your class, your request might not be fulfilled until the following week, depending on staffing needs.

Progress Evaluations are facilitated by either the Floor Manager on Duty or the Manager on Duty and are conducted while the student is swimming with their regular instructor during their perpetual lesson. We do not conduct Progress Evaluations during Makeup Lessons or Unlimited Swim Classes.

BABY SWIM: If you have any questions regarding moving your child from Baby Swim (ages 3 months to 36 months) up to Learn to Swim (ages 2 years to 12 years), please email Angela, our Baby Swim Department Leader, directly at babyswim@goldmedalswimschool.com

SWIM TEAM: If you have any questions regarding Swim Team (i.e. moving your child's booking to or from one of our off-site swim teams), please email Shelby, our Swim Team Department Leader, directly at swimteam@goldmedalswimschool.com

NEW! All Learn to Swim level progression is now updated and recorded weekly! Please first check your UDIO Customer Account to review your swimmer's progress prior to submitting a Progress Evaluation request. Learn more HERE!

I agree to the information above regarding Progress Evaluations.

I have reviewed my LTS swimmer's progress in my Udio Customer Account.

I understand I may request a Progress Evaluation ONCE every three months.
