Here at the Gold Medal Swim Club, your swimmer is our priority. If you’re taking a break, moving, completing our program, or stopping for health reasons, please complete this withdrawal form.

Notice Required:
• Swim Club: 30 days
You may choose any future cancellation date.

Processing:
Cancellations are accepted only through this form. You will receive an email confirmation, then a notice with your final class date. Your spot will not be held—re-enrollment depends on availability, and many classes have waitlists.

Tuition & Fees:
• After the notice period, tuition stops.
• If your last class is mid-month, tuition is prorated.
• Requests before the monthly debit run are charged only for scheduled lessons; after, any balance becomes Family Credit (except for Club swimmers).
• No refunds for past lessons.
• Annual registration fees apply per calendar year; re-enroll within the same year to avoid re-assessment.
